Division Dlrec[or <br />Natural Resource Trustee <br />Henderson, CO 80640-8621 , ~ <br />Re: 124th Estates Sand and Gravel Mine, Permit No. M-2001-085, Reclamation Responsibility Release <br />Request, SI.-1 ,/ <br />Deaz Mr. Ewegen: <br />On May 18, 2004, the Mined Land Reclamation Division received your request for full reclamation responsibility release <br />for the 124th Estates Sand and Gravel Mine, Permit No. M-2001-085. Preliminary review of your request determined <br />that the following information must be submitted to complete the requirements of Rule 4.16 of the Construction Material <br />Rules and Regulations and C.R.S. 34-32.5-117(5) of the Colorado Land Reclamation Act for the Extraction of <br />Construction Materials: <br />4.16.1(2) The Operator shall include in the notice to the Office the names and addresses and phone numbers of <br />all owners of record to the affected land. <br />4.16.1(3) The written notice requesting release shall be sent by Certified Mail and be separate from other types <br />of communication to the Office. <br />4.16.1(4) Such notice shall contain a signed statement by the Operator or their agent that all applicable portions <br />of the Reclamation Plan requirements have been satisfied in accordance with these Rules and all applicable <br />requirements under the Act. <br />If you have any questions, please do not hesitate to call me at 303-866920. <br />Sincerely, <br />.1 _` <br />Suzi Eri n <br />Program Assistant <br />cc: Tony Waldron, DMG <br />Office of <br />Mined Land Reclamation <br />Office of <br />Active and Inactive Mines <br />Colorado <br />Geological Survey <br />
