ADDENDUM 3 <br />• Sedimentation Pond 006 - Phase 11 <br />As-BUilt Certification <br />Introduction <br />The embankment and spillway structures of Sedimentation Pond 006 - Phase I was constructed <br />in the fall of 1989 and according to the designs contained in the approved permit. The <br />as-built conditions of these structures are shown in Exhibit 13-6. This certification <br />covers the Sedimentation Pond 006 - Phase Il. <br />Construction Summary <br />In accordance wiTh Rule 4, Part 4.05.6(7) of the Rules end Regulations of the Colorado <br />Mined Land Reclamation Board pursuant to the Colorado Surface Coal Mining Reclamation Act <br />34-33-101 et seq., I, Ronald J. Gehrke, hereby certify that the construction of <br />Sedimentation Pond 006 - Phase II was inspected and constructed under my supervision. <br />• From September 20 until September 21, 1989 during Phase 7 construction, 1 was on site to <br />inspect the construction of this structure. The construction of Phase 1 was supervised on <br />site by Rey Armijo, a graduate civil engineer, and Chuck McCulloh, Operations Engineering <br />Manager at the Seneca 11-Y Mine. Rey end Chuck assured all cork was performed in <br />accordance with the design and the specifications. 1 inspected the Phase 1 structure on <br />November 7, 1989, May 22, 1990, June 18-20, 1990, end again on July 27, 1990 to verify the <br />ponds conditions. 1 also inspected the completed Phose 1 6tructure on September 5, 1990 <br />and again on November 19-20, 1990 to verify the es-built conditions. I inspected the <br />Phase 11 construction on August 1, 1991 end again on November 21, 1991 to verify the <br />as-built conditions. 1, Ronald J. Gehrke, em a 5977 graduate in civil engineering and I <br />have been a Registered Professional Engineer in Colorado since November 22, 1982. <br />The purpose of my inspection end certification was to ensure proper construction of <br />Sedimentation Pond 006 - Phase II end to satisfy the requirements of Rule 4.05.6(7) of the <br />Colorado Mined Land Reclamation Division. The material was placed in the embankment to <br />meet or exceed the design specification (see Addendum 4 for compaction inf ormacion). This <br />as-built report confirms the construction of this structure meets or exceeds the Rules and <br /> <br />
