Long <br />Division Director <br />Rapid City, SD 57709 <br />RE: Rex Quarry -Sec 36, Amendment Approval, Permit No. M-77-304, Revision No. <br />AM-2 <br />Deaz Mr. Grotenhouse: <br />On February 6, 1997 the Division of Minerals and Geology approved the Amendment application <br />submitted to the Division on October 23, 1996, addressing the following: <br />Construction of the tie-across haul road to the Roberts Quarry. <br />The terms of the Amendment No. 2 approved by the Division aze hereby incorporated into Permit <br />No. M-77-304. <br />The conditions of the Division approval are noted below. Please note that all other provisions <br />of Permit No. M-77-304 remain in full force and effect. <br />Stipulation No. Description <br />1. The operator must submit the required reclamation bond increase of $10,860.00 <br />within 60 days of approval of this amendment, only if no additional changes occur <br />as a result of the reclamation liability adequacy inspection which will be <br />conducted on February 19, 1997. If the scheduled inspection reveals that <br />additional bond will be required for the entire site, the amount of bond submitted <br />shall be commesurate with the additional increase as determined by the inspection <br />and subsequent reclamation bond re-calculation. <br />11~1~I1~ ~~~I~~ <br />STATE OF COLORADO <br />
