Loos <br />Division Dueaor <br />Re: Pit 1, File No. M-99-073, Receipt of 110 Construction Materials Reclamation Permit Application Package <br />Dear Mr. Melbv: <br />On September 13, 1999, the Division of Minerals and Geology received your I l0 Conswction Materials Reclamation Permit <br />Application package and a few of the required Exhibits for Pit 1, File No. M-99-073. Preliminary review of the information <br />received determined thaz one additional copy of the corrected appkcation form plus another copy of Exhibits B, C, D, F, and H need <br />to be submitted to the Division, and two (2) copies of the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM: Page 1 -Item 3: If you are going to mine more than 9.9 acres (this is the maximttm acreage <br />amount allowed for a Conswction Materials Limited Impact 1 LO Operation) then you will need to submit one original <br />and (four) 4 copies of the enclosed Conswction Material Regular 112 Operation application plus five (~) copies of all <br />of the required Exhibits and an additional application fee in the amount of $1,000.00. If you are going to mine 9.9 acres <br />please correct Item 3 Acreage amount. <br />APPLICATION FORM: Page 6 - Since yolu company is a Corporation please have your Corporate Secretaq~ sign and <br />affix Evan L. Melby, Inc.'s Corporate Seal on Page 6. Also, please have Lorraine C. Medina complete the notarization <br />of your signattue on Page 6. <br />EXHIBff A -Lee Description (Rule 6.3.1(11): The legal description must identify the affected land, specify affected <br />areas and be adequate to field locate the property. Description shall be by township, range, and section, to at least the <br />nearest quarter~lrarter section. A metes and bounds survey description is also acceptable. Also, the street address or lot <br />number(s) may be used. This information is available from the Counh Assessor's office or U.S. Geological Survey <br />(USGS) maps. <br />EXHIBIT A -Location Map (Rule 6.3.1(211: A map showing information sufficient to determine the location of the <br />affected land on the ground and existing and proposed roads or access routes to be used in connection with the mining <br />operation. Names of all immediately adjacent surface owners of record shall also be shown. The operation location map <br />shall be a standard 1:24,000 scale U.S. Geological Survey map. The location of the proposed operation shall be shown <br />and labeled with the mine site name. <br />EXHIBIT B -Site Description (Rule 6.3.2fa11: You must address Item (a) to the extent necessary to demonstrate <br />compliance with the applicable performance standard requirements of Rule 3. At a minimum. you must include a <br />description of the soil characteristics in the area of the proposed operation. The local office of the Natural Resources <br />Conservation Service (MRCS) may provide you with this information as well as recommendations for Exhibit D - <br />Reclamation Plan: <br />
