Klco <br />10494 Highway 50 Azurite, Inc. <br />Howazd, CO 81233 Cotopaxi, Colorado 81223 <br />(719)942-3288 (7l9) 942-4178 <br />In the course of the reconstruction of State Highway SOA, it is necessary to issue a Temporary Access Permit to your existing <br />access to the State Highway in the interests of the public health, safety and welfare; and to bring it into conformance, to the <br />extent feasible, with the State Highway Access Code. Such issuance is necessitated by changes traffic volumes. The access <br />shall be used in accordance with the slate highway access statute, the State Highway Access Code, the terms and <br />conditions of this permit including those on page 2 of this form and those special conditions as follows: <br />Special Conditions: <br />1- The temporary access is located on the south side of SH 50 at milepost 249.663. <br />2. The temporary access is for the purpose of providing aggregate base course and aggregate for Hot Bituminous <br />Pavement for CDOT Project NH 0503-058. <br />3. The temporary access permit is issued for the duration of the construction oT the project. The increased traffic <br />volume is allowed for the benefd of the CDOT Project only. This permit expires upon the completion of the CDOT <br />construction. <br />You are invited to submit any written data, views, arguments and objections in regards to this action. You may also request a <br />hearing. Should you choose to object to this permit or any of the terms and conditions of this permit or request a hearing on <br />such objection, you must do so in writing on or before May 5, 2000.
