These are as follows: I. Map C-4 needs to be updated showing the location of all structures, ditches, culverts etc. 2. Section 2.04.6 pg 4: the rewrite of the page has left off the beginning of the last paragraph from [he prior edition. "An inspection....." 3. Section 2.05.3 page 3: same as above except it is the fits[ paragraph starting with "(storage) area, a main fan..." 4. Section 2.0.5 page I: the official land use statement should include "rangeland and wildlife habitat". 5. Using the new list of corporate officers and directors, the Division conducted an AVS check for the National King Coal Mine (C-81-035). The system recommendation is 'conditional issue.' There appears to be a state civil penalty and bond forfeiture associated with West Virginia permit U400988T covered under an existing settlement agreement. Please see attached documentation for further detail. The following question needs to be clarified by the operator; There are numerous officers and directors associated with National King Coal, LLC., American Metals & Coal International, Inc. and KM Investments Corporation that are listed in the Applicant Violator System (AVS) but are no[ included in the permit pages. The following officers were noted in the system;
