As we discussed <br /> in our phone conversation, the Division believes that this permit change <br /> should be submitted as an amendment (please refer to Rule 1 .8) . Enclosed is a <br /> 112 application form, which should be completed and submitted with the <br /> amendment. <br /> Please review your original permit application to determine which Exhibits <br /> will require changes and updated information. If there are no changes for a <br /> particular exhibit, please state so, and refer to the original permit <br /> application. <br /> After review of the submitted technical revision, I would like to suggest the <br /> following items to be considered for an amendment: <br /> 1 ) Investigate the feasibility of constructing a cyanide tailings <br /> disposal site. The site should be sized for the anticipated amount <br /> of tailings and/or slurry, lined and ditched or bermed to divert <br /> storm and snowmelt runoff. Final reclamation should include <br /> covering with adequate depths of growth medium and reseeding. <br /> 2). Submit a plan for disposal of cyanide drums. EPA has verbally <br /> recommended to triple rinse, remove labels, crush and dispose of in <br /> a land fill . <br /> e <br /> 3) For most cyanide operations the Division has recommended to the <br /> Board a stipulation regarding a monitoring program.
