The annual <br />fee violation M-90-098 has now been abated and no further action will be taken <br />to revoke your permit, Please make note of your anniversary date of October 6 <br />for the 1991 reporting period. The annual fees and reports are due on or <br />before your anniversary date each year, until you are release of all <br />reclamation responsibility and your bond has been released in its entirety, to <br />avoid violations, civil penalties, and possible revocation of your Dermit. <br />I have enclosed an annual report form for your use.
