Box 1029 <br />Grand Junction, CO 81502 <br />RE: Our Files M-78-286 - Urantah Mine, <br />M-80-55 -Topaz Mine <br />M-78-39 - St. Jude Mine <br />M-77-416 - Carnation Mine <br />Dear Mr. Haubold: <br />On September 26, 1985 the Mined Land Reclamation Board approved the transfer <br />of the above permit by succession of operator from Pioneer Uravan, Inc. to <br />Umetco Minerals Corporation. <br />In order to allow the Division to complete the transfer, please complete the <br />following forms and return to the Division as soon as possible: <br />Exhibit A - Legal description for financial and performance <br />warranties must be the same as in Exhibit A of each permit <br />(enclosed). Also, permit acreage must be the same as that <br />permitted as described on line 12 of the original application form <br />for each permit (enclosed). <br />2. Please complete an application form for Umetco for each permit <br />(enclosed). <br />Upon receipt of the above forms (complete and adequately filled out), the <br />permits will be transferred and a copy of the original permit document, <br />succession of operators form and required warranties will be forwarded to <br />you.
