Long, <br />Division Director <br />Re: Receipt of Permit Conversion Application, Juniper Quarry, Permit No. <br />M-82-141, Revision No. CN-O1 <br />Dear Mr. Steele: <br />We have received your conversion application for the Juniper Quarry, Permit <br />No. M-82-141, in Moffat County. All comment and review periods began on the <br />date of submittal. The tentative decision date for your application is <br />May 26, 1992. The Division requests that before the decision date you submit <br />a copy of the access agreement that Moffat Limestone Company has with Urie <br />Ranches for useage of the haul road across the ranch property. <br />Within ten (10) days after submittal, you must publish notice in a newspaper <br />of general circulation in the locality of the proposed mining operation once a <br />week for four consecutive weeks. Immediately after the first publication, <br />mail a copy of the notice to all owners of record of surface rights to the <br />affected land and all owners of record of lands that are within 200 feet of <br />the boundary of the affected land. Proof of the notice and mailings, such as <br />Certified Mail - Return R~aceipt Requested, must be submitted to the Mined Land <br />Reclamation Division prior to the decision date. <br />The Division is reviewing your application to determine whether it is adequate <br />to meet the requirements cif the Act.
