Section II -Revisions to the Permit <br />Subsequent to the permit renewal on November 16, 1991, P&M has submitted and a Division <br />decision has been made for the following revisions: <br />Minor Revision No. 27 was received on May 11, 1992. The revision sought approval to store <br />topsoil in windrows adjacent to the Moffat pit area. The revision was approved on May 27, 1992. <br />Minor Revision No. 28 was received on May 19, 1992. The revision restored language in the <br />permit regarding the winter contemporaneous reclamation variance. Though the approval had <br />already been granted, the language had since been inadvertently omitted from the permit. The <br />revision was approved on May 27, 1992. <br />Minor Revision No. 29 was received on June 22, 1992. The revision requested approval for <br />exploration drilling within the approved permit boundary, outside of the then approved <br />disturbance limits. P&M applied instead for an exploration permit, and subsequently withdrew <br />the revision application on July 13, 1992. <br />Minor Revision No. 30 was received on November 12, 1993. The revision incorporated text <br />describing the frequency of operator inspections of the West Ridge check dams. The revision was <br />approved on December 13, 1993. <br />Minor Revision No. 31 was received on Decerttber 3, 1993. The revision reinstated the <br />commitment to conduct quarterly inspections of all sediment ponds. The inspections were <br />previously conducted only semiannually, during the 2nd and 3rd quarters of the year. The revision <br />was approved on December 13, 1993. <br />Minor Revision No. 32 was received on February 9, 1994. The revision sought approval for <br />special handling of Moffat pit spoil, allowing for a delay in grading to accommodate drainage <br />patterns. The revision was approved on February 10, 1994. <br />Technical Revision 21 was received on April 26, 1993. The revision reclassified the North Access <br />Road as the North Haul Road. The revision was approved on June 7, 1993. <br />Technical Revision 22 was received on April 27, 1993. The revision provided a demonstration <br />that the drainage from the Trout Creek Pump Pad, through the use of alternate sediment control <br />measures, would meet effluent standards without being passed through a sedimentation pond. <br />The revision was approved on June 8, 1993. <br />Technical Revision 23 was received on May 17, 1993. The revision provided designs for the inlet <br />to the Moffat pond, in response to Notice of Violation No. C-93-025.
