The terms of the AM No. 02 approved by the Board are hereby <br />incorporated into Permit No. M-84-041. Please note that no disturbance may <br />take place on this area until the additional 542,750,00 bond is received. <br />The conditions of the Board approval arc noted below, Please note that all <br />other provisions of Permit No. M-84-041 remain in fuil force and effect. <br />1. As-built plans of all sediment ponds must be submitted to <br />the Division after construction is completed, <br />2.
