Midterm Response
Per your request, I conducted a midterm review of the Hayden Gulch
Mine (C-80-003). I reviewed the following sections; Identification
of Interest (2.03.4), Compliance Information (2.03.5), Right of
Entry and Operation Information (2.03.6), Operation Plan (2.05.2),
Sealing or Managing Mine Openings (2.05.4(2)(8)), Bonding &
Reclamation Cost Estimate (2.08.5 & 2.05.4(2)(b)) and Backfilling
& Grading (2.05.4(2)(c)). Based on this review, I have the
following concerns that should be addressed to the operator:
1. Section 2.03.4 of the regulations pertains to identification
of interest. The information contained in the permit dates
back prior to 1986, and appears to be obsolete. Please
provide a current description of the identification of
interests as required in Section 2.03.4 of the regulations.
Section 2.03.5 of the regulations pertains to compliance
information. The compliance information in the permit dates
back prior to 1986. Please update this information as needed.
In particular, section 2.03.5(3) requires that the applicant
submit a list of all violation notices received to date. The
violations listed in the permit date prior to 1986. Please
update the notice of violations received from 1986 to present.
