Permit No. C-048-81 <br />Dear Mr. Mills: <br />Enclosed please find the proposed settlement agreements for the above cited <br />violations. These proposals are based on the assessment conference held at <br />your mine on Tuesday, July 24, 1984. Subsequent to that meeting, you were <br />given the opportunity first to July 31, and then to August 7, to supply <br />additional information regarding the violations. Having received nothing, I <br />am proposing this settlement. In attendance at the assessment conference, in <br />addition to you and me, were your mine personnel and Jim Herron, Mike Savage <br />and Susan Mowry, from Division Staff. <br />If you agree with the proposed settlements, please sign the form and return it <br />to the office as instructed.
