All revisions ~ to <br />address the requirements of Ruk 2.OB.4. Three (3) copies of the revision, including maps, must be submitted in ordrs <br />for it to be complete. <br />AII revisions are to be formarted so they can be insened inro the permit to replace the revised sections, maps, tables <br />and/or figures, with a revised table of contents, i£ accessary, The revicioa subarittai dart should be printed in the <br />lower right comer of each revision page. A cover letter to the revision should explain the nature of nc~ revision and <br />reference the specific penult sections being revised. <br />For federal mines, a copy of the revision application must be aubmitced so sll agencies on the federal raailirlg list <br />(except 05141) at the same time nc~ application is aubmitu:d to the Division, and proof of distnbudon must be <br />submitted to the Division along with the application.
