<br />,, <br />Mr. Weaver <br />February 12, 1992 <br />A. The Division will need a topographic map showing the facilities area at iu present <br />topographic configuration. This is needed in order for the Division to determine the <br />amount of backfilling that will be required to cover the portals and portal bench. <br />B. This map, as with all maps submitted as part of the permit application, must <br />conform to the standards of Rule 2.10, and as such, must be prepazed by or under <br />the direction of, and certified by, a qualified registered professional engineer or <br />professional geologist. <br />C. The facilities should be shown to scale and labeled. The map also should show <br />roads, buildings, conveyors, concrete structures, portals, power lines, storage pad, <br />ditches, fuel tanks and all structures that must be removed during reclamation. The <br />waste pile should be delineated and show the present configuration. <br />D. The maps should contain a scale and north arrow. <br />E. The Division will need an inventory of the building, conveyor structures and fuel <br />tanks. The floor azea, height and type of construction for each building should be <br />listed. Also the Division will need to know the length of conveyors, size of concrete <br />footings and dimensions of structures. <br />F. What is the current size (volume) of the coal waste pile? <br />4. A public notice was not included with the permit renewal application. Rule 2.07.3(2) <br />requires the applicant to publish a public notice. This notice allows the public to participate <br />in the permit renewal process. A notice containing the required information is attached. You <br />must publish this notice once a week for four (4) consecutive weeks in a newspaper of local <br />circulation. After publication please send the Division the proof of publication. <br />5. The certificate of insurance in the current application has expired. Please submit a current <br />certificate of insurance to be inserted into exhibit 3. <br />6. A recent Regulation modification added Rule 2.04.13. This Rule requires the submittal <br />of an annual reclamation report. For an underground mine the informatio^ required is <br />minimal if you have not disturbed or reclaimed acreage in the past year. This information <br />is required, however. The Regulations require that this the mine submit the report by <br />February 15, of each year but you may submit it as part of the adequacy review response. <br />3 <br />
