(OCM) to Oxbow Mining, Inc. (OMI), along <br />with copies of all revised and updated permit materials and supporting documentation <br />associated with the permit transfer. <br />Specifically, this submittal includes the Proposed Decision Form for the permit transfer, <br />signed by the operator's authorized representative; acopy of the DMG's Findings; a copy of <br />the permit transfer application form; a copy of the bond form; a copy of the insurance <br />certificate; and copies of associated revised permit text and appendix pages.
