BOX 177 <br />Silverton, CO 81433 <br />Re: Receipt of 112 Hard Roclc/Metal Mining Reclamation Permit Amendment Application, <br />Sunnyside Gold Mine, Permit No. M-1977-378, AM - 5 <br />Deaz Mr. Perino: <br />On May 12, 2003, the Division of Minerals and Geology received your 112 Hazd Rock/Metal Mining <br />Reclamation Permit Amendment application for the Sunnyside Gold Mine, which is located in San Juan <br />County. All comment and review periods began on May 12, 2003. The decision date for your application is <br />scheduled for August 1 I, 2003. <br />Within ten (10) days after submittal, you must publish notice in a newspaper of general circulation in the <br />locality of the proposed mitring operation once a week for four (4) consecutive weeks. Immediately after <br />the first publicafion, mail a copy of the notice to all owners of record of surface rights, holders of any <br />recorded easements, and all owners of record of lands that are within 200 feet of the boundary of the <br />affected land. Proof of notice and mailings, such as Certified Mail -Return Receipt Requested, must be <br />submitted to the Division of Minerals and Geology prior to the decision date. Additionally, any changes or <br />additions to the application on file in our office must also be reflected in the public review copy which has <br />been placed with the San Juan County Clerk and Recorder. <br />The Division is reviewing your application to determine whether it is adequate to meet the requirements of <br />the Act.
