Mining Company, the original Operator of Record, to Eagle-Gypsum, <br />Limited, the operator de facto. R.A. Mining ~omipany a Colorado General <br />Partnership whose address is P.0. Box 488, Glenwood Springs, Colo., 81602 <br />(General Partners: Jerome F. Gamba, Kim C. Harden and Howard E. Tingley) <br />has been replaced by Eagle-Gypsum, Limited, a Colorado Limited Partnership <br />whose address is P.0. Box 488, Glenwood Springs, Colo., 81602 (General <br />Partners: Jerome F. Gamba, Kim C. Harden, Robert J. Lickus and Howard E. <br />Tingley). This change was required in order to raise capital necessary <br />to place the Eagle-Gypsum Mine into operation. <br />R.A. Mining Company has transferred their interest in the reclamation <br />cash bond (Certificate of Deposit No. 15962) to Eagle-Gypsum, Limited. <br />Eagle-Gypsum, Limited hereby assumes all liability for the reclamation S <br />of the affected land as pledged by the aforementioned Certificate of <br />Deposit. Further, Fagle-Gypsum, Limited includes with this request a <br />new Performance Warranty statement as required to be in compliance with <br />Mined Land Reclamation Division "Mineral Rules and Regulations". <br />I trust this request for transfer of permit, which is basically <br />a name change of the existing operators, may meet with your approval. <br />In addition, I wish to advise the Division of a change in Contact <br />~}du Person at the Fagle-Gypsum Mine. Mr. Jerome F. Gamba has been designated <br />Mine Manager; Further communication regarding the mine operation, status <br />of reclamation or other pertinent concerns should be addressed to Eagle- <br />~p Gypsum, Limited, Attention Mr. Jerome F.
