The terms of <br />the revision approved by the Division are hereby incorporated into <br />Permit No. M-88-112. <br />The conditions of Division approval are noted below. Please note <br />that all other provisions of Permit No. M-88-112 reIDain in full <br />force and effect. <br />STIPULATION #1 <br />THE OPERATOR WILL INSTALL AND MONITOR A MINIMUM OF TEN PIEZOMETERS, <br />WITH FIVE INSTALLED ALONG THE CREST OF THE EMBANKMENT RAISE AND <br />FIVE INSTALLED ALONG THE UPSTREAM TOE OF THE EMBANKMENrp RAISE. THE <br />PIEZOMETERS ARE TO BE INSTALLED AND MONITORED IN ACCORDANCE WITH A <br />DESIGN AND PROTOCOL TO BE APPROVED BY THE DIVISION. A DESIGN FOR <br />THE PIEZOMETERS MUST BE SUBMITTED FOR REVIEW AND APPROVAL BY <br />FEBRUARY 15, 1994. THE PIEZOMETERS MUST BE INSTALLED BY MARCH 15, <br />1994 AND TAILING SURFACE ELEVATION IN THE LOWER POND MAY NOT EXCEED <br />ONE FOOT BEYOND THE ELEVATION AT THE BASE OF RAISE 1 AT THE TIME OF <br />PEIZOMETER INSTALLATION. <br />STIPULATION #2 <br />THE OPERATOR WILL INSTALL MARKERS TO DELINEATE THE SET-BACK <br />REQUIRED BETWEEN THE TAILING EMBANKMENT AND THE FREE WATER POND. <br />THE MARKERS MUST BE CLEARLY VISIBLE AND ADEQUATE TO ALLOW STRAIGHT <br />FORWARD VERIFICATION IN THE FIELD THAT THE REQUIRED SET-BACK IS <br />MAINTAINED. <br />
