Cattany <br />Division Director <br />Natural Resource Trustee <br />The Division of Reclamation, Mining and Safety received the Technical Revision Application 03 (TR-03) to the <br />Three Bells Pit, Permit No. M-1979-191. The Division has the following items that need to be addressed prior to <br />approval of the revision. <br />1. Please provide a signed letter from Lafarge West, Inc. authorizing TST, Inc. Consulting <br />Engineers to submit TR-03 on their behalf. <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional engineer, or <br />other qualified person. The maps submitted to the Division were not signed. Please resubmit a <br />signed copy of the maps. <br />3. The map submitted with the application (titled Exhibit E) depicts an area that is not well defined <br />within the approved 360 acre permit area. The Division printed out the mining and reclamation <br />maps approved in 1979 and highlighted the approved permit boundary. The Division was unable <br />to determine from the map submitted with TR-03 where it relates to in the approved permit area. <br />Please submit a map or revise the map to identify the location within the approved permit area. <br />4. Exhibit E contains contour elevations for both existing and reclaimed areas. However, with both <br />contour lines on the map it is difficult to decipher which is which. In addition, the contour lines <br />on the northeast corner of the site stop midway and do not connect to any other contour lines. <br />Please submit a separate map depicting the existing contour elevations and the reclaimed contour <br />elevations. Please also include the contour elevations on both maps. <br />5. The operator notes a ditch re-aliglment on the western side of the map. Please state the ditch to <br />be relocated. If Lafarge intends to mine through and relocate a ditch, they will need to submit a <br />notarized agreement between the operator and the owner(s) of the ditch allowing this to occur. <br />Please submit a notarized agreement or comply with Rule 6.4.19 as it relates to mining within 200 <br />feet of permanent man made structures. <br />6. Please submit engineered drawings and designs of the re-aligned ditch. <br />7. Please locate on the map all existing permanent man made structures within 200 feet of the <br />affected land. In accordance with Rule 6.4.19, if mining or reclamation is to occur within 200 <br />feet of a structure Lafarge must either; <br />Office of Office of <br />Mined Land Reclamation Denver ~ Grand Junction Durango Active and Inactive Mines <br />
