W V Permit 5006280 was released on January 20, 1999. W V Permit 5001385T was released on December 1, 1999. W V Permit 0005485 was released on July 28, 1998. W V Permit 5308087 was released on October 29, 1998. The status of W V Permit 0302392 is unlrnown at this time. Munger Item 4. Items 6a through 9 on pages 8 through 10 of the revision application were evidently copied verbatim from the McClane TR-11 Appendix A submittal. These items are permit specific, and as such the information applies to the McClane permit, not the Munger peraut. Please amend Items 6a through 9 with the correct information for the Munger permit ResOOnse: Items 6a through 9 have been amended to apply to the Munger petmit and are being submitted as Munger Appendix A pages I I and 12.
