I have the following comments regarding [his permit revision application. <br />1. Wells DH-13 and DH-34B have been removed from the approved monitoring plan as <br />indicated on revised page 2.05-76. The 1999 Annual Hydrology Report (AHR) indicates that these <br />wells were destroyed by the mining operation. li` these wells are no longer needed, then they should <br />be properly sealed in accordance with Rule 4.07 and an abandonment report should be submitted to <br />the Division for each of these wells. <br />2. It appeazs that the nomenclature for wells DH-57a, DH-57b, and DH-S8a, DH-58b has been <br />changed to DH-57, DH-57a, and DH-58, DH-S8a, respectively. Please clarify this and ensure that <br />all maps, permit text, and AHR information reflect the correct nomenclature for these wells. <br />3. For the new wells added to the monitoring program (DH-65, DH-66, DH-67-Abv, DH- <br />67D, DH-67-D-Blw, DH-67-B) please update Volume III of the permit document with the <br />appropriate well completion diagrams for each of the new wells. <br />4. The Division is in the process of adopting changes to our surface and ground water <br />monitoring rules. The tole changes will incorporate compliance with applicable ground water <br />standards at established points of compliance. Additional information regardin downgradient <br />ground water quality may be requested at the time the new rules are promulgated~Mrdirtg~e-Ehe <br />h. ~ddi[ional groundwater monitoring wells may <br />be required to establish appropriate point(s) of compliance. <br />Please let me know if you require additional information. <br />M:coaUmpb/bowplO4 <br />
