Preliminary review of the information <br />received determined that the following items must be received before the Division can consider your application as being <br />subm~i"tted~and technical review can begin: <br />r~~/APPLICATION FEE: The Division did not receive the entire $2,344.00 application fee with fhe application <br />- package. <br />/ AYPLICAT[ON F012M: The Division requires use of an original application form. TFte Division requires that [he <br />Y Operation name be the same as the name on the pemvt (i.e. Funk & Hooker Pit). Please be sure that all the <br />Responsibilities as a Pemuttee (pages S and 6) and the Signature page (page 7) are completed. Please ensure that the <br />sigmattrre page is notarized by a Notary Public. <br />,~ ~XI-IIBIT K -Climate (Rule 6.4.11): Provide a description of the significant climatological Factors for the locality. <br />Yotrr application will not be considered submitted until the information listed above is received and found sufficient to begin <br />our review. A decision date will be established 90 days from the date of receipt of all of the requested information. <br />Additionally, if you have aheady published notice you will need to republish notice, after the Division considers the application <br />submitted. This notice must be published once a week for Four (4) consecutive weeks, starting within ten (10) days of the date <br />your application is considered submitted. We will notify you when you should initiate republication of your notice. The final <br />date for receiving cotnments is the 20th day after the fourth publication or the next regtilaz business day. <br />This letter shall not be constmed [o mean that there are no other technical deficiencies in your application.
