This revision cannot be approved until
the following adequacy issues have been adequately addressed. Please respond to these issues as
soon as possible, but no later than June 24, 2002 so the decision due date of July 1, 2002 can be met.
If your adequate response, or a written request for extension of the decision date is not received by
June 24, 2002, this application may be denied.
ADEQUACYISSUES
1. In order to leave the maintenance shop as a permanent site feature, documentation must be
submitted proving that the building meets the local building code.
2. Please specify how united proposes to reduce the slopes of, replace topsoil on, and revegetate the
island. Currently, the Division will assume that the lake will be dewatered so the island and north pit
slope can be accessed for reclamation. The bond will be recalculated accordingly.
3. DMG can approve a somewhat flexible final configuration for the backfill area of the site. However,
we are charged with ensuring adequate bond is held for reclamation of the site. Therefore, United
Companies must commit to a maximum area and depth to be backfilled (and illustrate on map) so
reclamation cost figures for this area can be calculated. If plans change in the future, another
revision can be submitted.
4. If the beach is a swimming area, please verify that the slopes in the beach area are no steeper than
the required 5H:1 V.
5. Using the TR-01 map and proposed plans, DMG has determined that costs will be included in the
reclamation estimate for reducing 3000 linear feet of pit slope to the approved 3:1 grade (including
700 feet of island slope). It is assumed that the slopes will average 1.5:1 prior to slope reduction.
6. Please revise the map to include all required details of Rule 6.12.1 and 6.4.6, specifically: the
approved permit boundary, roads, contour elevations, structures (water truck loading facility). Also,
