The Division, on July 12, 1996, received Savage and Savage's request for a reversal of the
Division decision to find Technical Revision TR-03 incomplete. The Division has considered this
request and herein denies it, as neither the application for Technical Revision TR-03, nor the
reversal request received on July 12, 1996, provided the following information:
1) A copy of the mine's Stormwater Management Plan;
2) Copies of the Final Approval versions of the mine's air emissions permits;
3) Copies of the mine's renewed well permits, or copies of correspondence indicating
the well permit renewal application process has been initiated by Oakridge Energy,
Inc.;
4) Information pertaining to the maximum, minimum and average surface water flow
of the Carbon Junction Canyon stream, as required by Regulation 2.04.7(2)(b)(i);
5) Information pertaining to the protection of the hydrologic balance, specifically with
regard to the protection of surface water quality:
6) A revised groundwater monitoring plan;
7) A revised determination of the probable hydrologic consequences:
8) Revised permit text paragraphs 4 of Page 5-l l and 2 of Page 5-13, addressing the
alteration of the Carbon Junction Canyon drainage way;
9) Revised permit text reflecting the three 24-inch culverts;
