The revised pages and map address the concerns stated in my letter dated
September 14, 1995. The operator only needs to send in the letter addressed to DOH
regarding the combined NPDES discharge points 001 and 007.
The operator requested that the Division clarify the need for an as-built map. The Division
only needs certification of the construction of the revised ponds as stated in Rule 4.05.6(10).
The term "as-built" was used rather loosely, meaning only a certification of the revised pond
by a P.E. The operator states that a certification of the revised ponds will be forwarded to
the Division as soon as all changes are completed. This certification statement will meet
the requirements of Rule 4.05.6(10).
v
c:\wp51\misc\951003
