Preliminary review <br />of the application packages determined that the following information must be <br />submitted before technical review of your application can start: <br />Application Form Pa4e 4 - Responsibilities as a Permittee: This section of <br />the application form was not completed. Please read and initial each <br />requirement on the enclosed page 4 to acknowledge that you understand your <br />obligations as the permittee, then return to the Division. <br />You may amend your application by submitting the requested items. In accordance <br />with Rule 1.8, a new decision date will be established 90 days from the date of <br />receipt of all requested items. If you have already initiated publication, you <br />will need to republish notice which specifies the final date for receiving comments <br />is the 20th day after the fourth publication or the next regular business day. <br />This letter shall not be construed to mean that there are no other technical <br />deficiencies in your application.
