Zhe violation was terminated June 8, 1992 following repair of the outlet and sukmittal of the required repair certification. An assessment conference was held August 26, 1992 aryl resulted in a reoamiendation to vacate the NOV. 4he NOV was formally vacated September 1, 1992. NOV C-92-035 was issued November 24, 1992 for failure to protect topsoil from wig and water erosion and contamination which sxxrld lessen the capability of the material to support vegetation. 'Ifie violation was partially abated as of January 15, 1993 and a request to extend the abatement period was suhnitted to the Division. Revised permit application pages sukmitted with this application include Volume 1, Section 2.03, pages 1 arxi 3 through 16. 'These pages were revised to update the permit application to reflect the current format for Rules 2.03.1 through 2.03.5.
