Long <br />Dwision Director <br />This letter is being written to address technical issues which must be resolved before the Division <br />of Minerals and Geology (DMG) can approve the application submitted by your company for a <br />change in status to your 110 reclamation permit, Permit No. #M-94-117, Gold Hill Mill. Please <br />address the following concerns prior to the due date of October 8, 1999. <br />1. In accordance with Rule 1.13.5(2)(c ), please describe the circumstances under which mining, <br />milling or reclamation would be resumed at the site, and the steps which would be taken to notify <br />DMG of the re-starting of production or reclamation operations. <br />2. Stormwater control: Please specify the pond surface levels at which various stages of <br />stormwater control (assembling of pumping equipment, pumping to land application, pumping <br />into the mill for emergency storage) would occur, and describe the actions and personnel <br />involved in controlling the volume of water in the pond. Also, please commit to notifying DMG <br />when pumping operations are initiated and completed. The attached volume-depth curves from <br />the 1998 pond design may be used to set these limits. <br />3. Hazel A adit control: Please specify the levels in the Hazel A mine adit which would prompt <br />pumping of water back to the pond, the personnel involved, and the notifications which would be <br />made to DMG and the Colorado Department of Public Health and the Environment (CDPHE). If <br />the Hazel A adit is to be removed from the zero discharge site plan, DMG must be notified. <br />4. Erosion control: [t has been observed that the silt fence on the downstream face of the dam is <br />inadequate erosion control, and a verbal request was made to DMG to allow a trench to be dug at <br />the toe of the dam in order to control the sediment runoff.
