o; <br />Revised 11/1/94 <br />3. NOV C-93-072 issued by CDMG on May 7, 1993 for failure to <br />protect stockpiled topsoil from erosion. Abatement <br />called for regrading a portion of a topsoil stockpile. <br />Civil penalty of $1,150.00 was agreed upon per settlement <br />agreement dated August 3, 1993 after the August 17, 1993 <br />assessment conference. <br />4. NOV c-93-073 issued by CDMG on May 7, 1993 for failure to <br />construct appropriate sediment control measures and <br />failure to pass drainage through a sediment pond. NOV <br />resulted from a snowdrift plugging a ditch. Civil <br />penalty of $1,150.00 was agreed upon per settlement <br />agreement dated August 31, 1993 after the August 17, 1993 <br />assessment conference. <br />5. NOV C-93-077 issued by CDMG on May 18, 1993 for failure <br />to pass disturbed area runoff through a sediment pond. <br />The NOV resulted from damage to sediment control ditches <br />resulting from a storm event exceeding ditch design by at <br />least a factor of 10. Civil penalty of $500.00 was <br />agreed upon per settlement agreement dated August 31, <br />1993 after the August 17, 1993 assessment conference. <br />6. NOV C-92-030 issued by CDMG on October 22, 1992 for <br />failure to retain a sketch of blast patterns. Civil <br />penalty of $150.00 was agreed upon per settlement <br />agreement dates January 8, 1993 after the December 23, <br />1992 assessment conference. <br />
