Response to Minor Revision 97-148 <br />Dear Ms. Binns: <br />Enclosed are three (3) copies of the above-referenced minor revision. The submittal of July 21, 1997 was in response to the <br />comments included in the Approval Form for MR 97-148 and was filed by the DMG as a new Minor Revision. MR 97-151. In order <br />to keep records straight, the following changes have been made: <br />• The Application Form has been updated and shows the change in disturbed area due to re-alignment of the access road from <br />the approved alignment from MR 97-148. <br />Pages 2.05-23(0), 23(al), and 23(b) have been modified to reinsert material omitted from the approved permit page 2.05- <br />23(a): sub-paragraphs "Batch Weigh/Loadout Building Access Road" and 'Waste Pile Access Road". The footer has been <br />revised to indicate MR 97-151 and dated August 12, 1997. <br />• Pages 2.05-54(f)(68) and 2.05-54(f)(68a) have been updated to indicate MR 97-151 and dated August 12, 1997. <br />The bond calculations were adjusted in the July 21 submittal from the original submittal of MR 97-148 to match the <br />methodology used by the DMG included with the Approval Form. The July 21 submittal also described the changes in the access <br />road alignment from the original submittal of MR 97-148. The approved bond amount for MR 97-148 was $6,118.00, based on <br />DMG calculations. Due to the changes in the alignment of the access road and inclusion of an administration fee of 17%, the <br />bond calculated for MR 97-151 is $7,355.00 for the same activities. Because MR 97-148 has been approved with a bond total of <br />$6,118.00, the change in bond for MR 97-151 will be the difference in bond amounts between the MR 97-151 and MR 97-148 of <br />$1,237.00. <br />Thank you for your cooperation in this matter.
