In comparing the Identification of Interests information in the permit application versus the same
information in the Applicant Violator System (AVS), the Division has found some apparent
discrepancies. The Division has the following questions or comments:
1) In the AVS, Raymond E. Dymond is listed as Executive Vice President, with a begin date
of 3/6/96. However, in the permit application, under Rule 2.03.4, Identification of.
Interests, Raymond E. Dymond is not listed. Please either document Raymond E.
Dymond's end date as Executive Vice President so AVS can be updated or submit the
appropriate revised permit application page adding Raymond E. Dymond as Executive
Vice President.
2) In the AVS, Larry A. Pryor is listed as Executive Vice President, with a begin date of
3/6/96. However, in the permit application, under Rule 2.03.4, Identification of Interests,
Larry A. Pryor is not listed. Please either document Larry A. Pryor's end date as
Executive Viec President so AVS can be updated or submit the appropriate revised
permit application page adding Larry A. Pryor as Executive Vice President.
3) In the AVS, Jay Patrick Bruton is listed as Vice President, with a begin date of 3/6/96.
On the submittal, Jay P. Bruton is not listed. Please either document Jay Patrick Bruton's
end date as Vice President so AVS can be updated or submit the appropriate revised
permit application page adding Jay Patrick Bruton as Vice President.
4) In the AVS, Lawrence H. Black is listed as Vice President, with a begin date of 4/1/92.
On the submittal, Lawrence H. Black is listed as Executive Vice President, with a begin
date of 3/6/96. Please document which position and begin date is correct for Lawrence
