Lyons Quarry, Permit No. M-19'77-208. The Division only received one copy of this Technical <br />Revision request. As of May 3, 2002, the Division requires Operators submit two copies of technical revisions and <br />other documents supplied to the Division (see DMG Bulletin -May 3, 2002 enclosed).
