Notify the BLM and EPA of the results of the evaluation and the proposed <br />' corrective actions to be taken. <br />5.9.3 Process Monitoring <br />' If changes in physical parameters at the production stream indicate a formation failure, <br />cavity breach, or production well breach, the BLM and EPA will be notified within the next BLM <br />t working day and the following procedures will be initiated. <br />1. The well pressure, temperature, and flow rate instrumentation will be checked for <br />' accuracy. <br />2. The well valves will be checked for possible leakage. <br />' 3.
