The total area of pond surface exposed after December 31, 1980 for each of the pits <br />shall not exceed those values listed in Table 1 of this approval. Should the total surface <br />area exposed exceed those amounts, an amendment will need to be filed with this <br />office. The total amount of ground water to be consumed at the gravel mining <br />operations shall not exceed the values listed in Table 3 of this approval. Should the <br />amount of ground water to be appropriated from each of the pits exceed the values <br />listed in Table 3, an amendment will need to be filed with this office. <br />4. The applicant is seeking to claim credit for water returned to the stream system from <br />dewatering at the Hazeltine pit that is in excess of the depletions caused by dewatering. <br />Prior to claiming any credit for the water returned to the stream system from dewatering <br />the applicant must install an adequate measuring device to measure the quantity of <br />water being pumped for dewatering purposes. <br />5. Aggregate's Fulton Ditch augmentation station must be equipped with an adequate <br />measuring device, specifically a Parshall Flume or similar device, acceptable to the <br />Water Commissioner. The installation of any device used for measuring replacement <br />water in this plan must be inspected by a Division 1 hydrographer. Furthermore, the <br />exact location of all return structures utilized in this plan must be provided to the Division <br />Engineer and Water Commissioner in order to allow for inspections of the structures. <br />Finally, all charts or other output data associated with each measuring devices utilized in <br />this plan must be provided in a timely manner acceptable to the Water Commissioner. <br />6. The Water Commissioners responsible for the administration of this plan are Robert <br />Stahl, George Varra, Scott Edgar, Bob Carlson and Mark Trivisonno. The applicant shall <br />maintain daily records of all diversions, replacements, and the amount of water used for <br />each particular purpose. The applicant shall provide a report of these records to the <br />division engineer and the water commissioners on a monthly basis on a form approved <br />by them. The accounting must be submitted within thirty (30) calendar days of the <br />end of the month for which the accounting is being made. The applicant has not <br />been providing accounting for this plan to the Water Commissioners in a timely <br />manner which prohibits proper administration of this plan. Failure of the <br />applicant to provide timely accounting to the water commissioners for the <br />remainder of this plan period may result in revocation of this SWSP. <br />in addition, the applicant shall submit a report from each entity making replacements; for <br />this plan, those entities are the City of Westminster, City of Thornton and Centennial <br />Water and Sanitation District. The report shall include an accounting of all wastewater <br />plant water effluent controlled by each entity, showing the total volume of water under its <br />control at the various wastewater treatment plants, and the amount committed to each <br />of the recipients of the water. <br />9. All releases of replacement water must be sufficient to cover all out of priority depletions <br />and be made under the direction and/or approval of the Water Commissioner(s). <br />10, All parcels of dried up land used to generate augmentation credits will be inspected <br />during the irrigation season to verify dry-up. The final verification of dry up will be in the <br />form of an affidavit signed by an individual having personal knowledge of the dry up for <br />this year's entire irrigation season for each parcel of land used in this SWSP. Atl <br />affidavits must be provided to the division engineer by December 15, 2006 in order that <br />the final determination of augmentation credits for the irrigation season can be made <br />
