activity which may result in noncompliance with <br />permit requirements. <br />(b) Compliance Schedules. Reports of compliance or <br />noncompliance with, or any progress reports on, <br />interim and final requirements contained in any <br />compliance schedule of this permit, shall be <br />submitted no later than thirty (30) days <br />following each schedule date. <br />(c) Twenty-four Hour Reporting. <br />(i) The operator shall report to the Director <br />any noncompliance which may endanger the <br />health of persons or allows the movement of <br />fluids containing any contaminant into <br />USDWs. Information shall be provided orally <br />within twenty-four (29) hours from the time <br />the operator becomes aware of the <br />circumstances, by telephoning EPA at <br />303.312.6211 (during normal business hours) <br />or at 800.227.8917 (for reporting at all <br />other times). The following information <br />shall be included in the verbal report: <br />(A) Any monitoring or other information <br />which indicates that any contaminant <br />may cause endangerment to an <br />underground source of drinking water. <br />(B) Any noncompliance with a permit <br />condition or malfunction of the <br />injection system which may cause fluid <br />migration into or between underground <br />sources of drinking water, <br />(ii) A written submission shall also be <br />provided within five (5) days of the time <br />the operator becomes aware of the <br />circumstances. The written submission <br />shall contain a description of the <br />noncompliance and its cause; the period of <br />noncompliance, including exact dates and <br />times, and if the noncompliance has not <br />been corrected, the anticipated time it is <br />expected to continue; and steps taken or <br />planned to reduce, eliminate, and prevent <br />recurrence of the noncompliance. <br />Page 30 of 120 <br />EPA FINAL Area Permit No. CO3870-00000 <br />
