within thirty (30) days after the end of the quarter. <br />Appendix B contains Form 7520-8 that may be copied and <br />used to submit the quarterly summary of monitoring. <br />E. PLUGGING AND ABANDONMENT <br />1. Notice of Plupgino and Abandonment. The operator <br />shall notify the Director forty-five (45) days before <br />abandonment of the well. When a Class III well is <br />retired from service, the borehole and casing shall be <br />securely plugged to prevent any movement of the fluid <br />into or between USDWs. <br />2. Pluocina and Abandonment Plan. The plan shall meet <br />the requirements of 90 CFR Part 196.10 and must <br />address the needs of any post plugging monitoring or <br />cleanup for the protection of USDWs. Individual wells <br />shall be plugged and abandoned upon retirement. <br />Plugging and abandonment will take place once it has <br />been determined that there shall be no potential <br />further use of a well, either for mining, testing, <br />monitoring, or other purpose, or if deemed necessary <br />for protection of USDWs by the Director. Prior to <br />plugging and abandoning, the tubing system shall be <br />removed from the long string casing. <br />The plugging and abandonment plan (Appendix C), <br />submitted by the operator and revised by the EPA, <br />consists of one (1) plug with the following <br />specifications. A cast iron bridge (CIBP) shall be <br />set inside the long string casing at approximately the <br />bottom of the casing. Natural Soda will establish a <br />cement plug from immediately above the CIBP to the <br />surface. If it is not possible to run a CIBP inside <br />the long string casing due to well irregularities, a <br />cement basket shall be run in the hole to establish <br />the cement plug at the bottom of the long string <br />casing, then taq it, and then cement from the basket <br />established plug to surface. Both the surface and <br />long string casings shall be cut down to 1 to 3 feet <br />below ground level to salvage the wellhead. Cutting <br />down the casings will also allow for more complete <br />surface reclamation, which was requested by the BLM. <br />These plans have been revised and approved by the EPA <br />and are consistent with UIC requirements. <br />The cement plug shall be set using one or more of the <br />approved methods described in 90 CFR Part 146.10.
