PART I <br />Page 2 <br />Permit No. COG-850000 <br />A. COVERAGE UNDER THIS PERMIT <br />1. This general permit authorizes discharges that consist entirely of <br />precipitation-induced surface runoff from coal mining and processing <br />operations and loadoucs, while active mining is taking dace or during <br />reclamation, into waters of the State of Colorado. Exceptions are <br />chose facilities in the Cherry Creek Basin upstream of Cherry Creek <br />Reservoir and those tributary to Chatfield Reservoir downstream of the <br />USCS gage at Waterton or in the Plum Creek drainage. Yo mine <br />drainage, preparation plant water, spoils spring water, domestic <br />sewage or ocher wastewater other than surface runoff may be <br />discharged. Multiple discharges from one facility may be authorised <br />through this permit but ALL DISCHARGES FROM A FACILITY COVERED BY THIS <br />PERMIT MUST CONSIST ENTIRELY OF SURFACE RUNOFF. If a facility has <br />discharges from one or more of the above excluded sources alone or in <br />addition to surface r+inoff discharges, an individual permit is <br />required. <br />2. In order to apply for certification under this general permit, the <br />owner, operator, and/or authorized agent of the subject facility shall <br />submit by certified mail or hand delivery, the completed Coal Mining <br />Wastewater Discharge Application (furnished by the Division) which <br />requires the following information (at a minimum): <br />a) Na.~e, address and descriptive location of the facili*_y; <br />b) Name of principal in charge of operations; <br />c) Name of the water receiving the discharge; <br />d) Description of the activity including the anticipated duration of <br />activity and/or discharge, anticipated volume, and rate of <br />discharge; <br />e) Description of any waste water containment and/or treatment; <br />f) A map showing the general geographical location of the facility <br />and a sketch of the facility showing outfall(s) and receiving <br />water(s); and <br />g) An analysis of the water Co be discharged. <br />Ac least thirty (30) days prior co the anticipated dace of discharge, <br />such information shall be submitted co: <br />Colorado Department of Health <br />:lacer Quality Control Division <br />P=_rmits and Enforcement Section <br />4210 East 11th Avenue <br />Denver, Colorado 80220 <br />The Division shall have up to thirty (30) days after receipt of the <br />above information to request additional data or deny the authorization <br />to discharge.
