Identification, Classification and Standards <br />1. IdentiTtcntion: The discharger from this facility go to Ennis Draw, Box Elder Creek and then the South Platte <br />River, which is within Segment 3, Middle South Platte River Sub-basin, South Platte River Basin. <br />2. Clnssihcntion and Antideerndntion Review: Segment 3 is classified jot the following uses: Use Protected; <br />Recreation, Class 2; Aquatic Lije, Class 2 (warm); Agriculture. The Division reviewed this facility on <br />3/13/95, and deternuned that nn nntidegrrulation evaluation was not required because the receiving water is <br />use protected. <br />3. Numeric Standards: The complete lis[ of standards for segment 3 which have been assigned in accordance <br />with the above class cations can be fourul in 3.8.0 Class~carionc and Numeric Standards for the South <br />P[ntte River Basin (S CCR 1002-8J. <br />IV. FAC/LITYAND WASTEWATER TREATMENT DESCRIPTION <br />A. FnciliN Description: This facility has been partially rrclnimrd rind some surfaces are revegetating. Because of <br />incomplete reclamation, nn active mining .status for the purpose ojthic permit will be used. When all areas that <br />drain to sediment pond p2 are revegetated, the permittee can request coverage under the post-mining limitations. <br />No mining is occurring nt this site. Activities are limited to reclamation and Jly ash disposal. <br />8. Bulk Stornee of Diesel (uel. Gasoline. Solvents or other Hazardous Materials: These are noted on page 12 of the <br />application, in appendix A, and on site sketch. <br />C Other Permits: These are listed on pages 5-7 ojthe application. <br />V. TERMS AND CONDITIONS OF PERMIT <br />A.
