• ART I <br />Page ld of 19 <br />Permit No. CO-0024562 <br />Sampling by the permittee for compliance with the monitoring requirements <br />specified above shall be performed at the following location(s): Outfall 001, <br />the discharge of mine drainage from the Carlton Tunnel bypassing the settling <br />ponds, or 002, the discharge of mine drainage from the Carlton after flowing <br />through a series of settling ponds and monitored adjacent to and upstream from <br />Teller County Road 88 prior to discharging to Fourmile Creek. <br />2. Monitoring Exemption or Modification <br />Effluent monitoring is required during periods when the operation can be <br />accessed. If the operation is not :safely accessible due to weather conditions, <br />then this shall be reported. <br />3. Reporting of Data <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a <br />monthly basis. <br />Monitoring results shall be summarized for each month and reported on Division <br />approved discharge monitoring report forms postmarked no later than the 28th day <br />of the following month. If no discharge occurs during the reporting period, "No <br />Discharge" shall be reported. <br />Duplicate signed copies of the above report forms shall be submitted to the <br />following addresses: <br />Colorado Department of Health <br />Water Quality Control Division <br />4210 East 11th Avenue <br />Denver, Colorado 80220 <br />Attention: Permits and Enforcement <br />U.S. Environmental Protection Agency <br />Denver Place <br />999 18th Street, Suite 500 <br />Denver, CO 80202-2466 <br />Attention: Water Management Division <br />NPDES Branch 8WM-C <br />4. Chronic WET Testing - Outfalls 001 and 002 <br />(a) Testing and Reporting Requirements <br />Tests shall be done at the frequency listed in Part Z.B.1. Test results <br />shall be reported along with the Discharge Monitoring Report (DMR) <br />submitted for the reporting period during which the sample was taken. <br />(i.e., WET testing results for the first calendar quarter ending March 31 <br />shall be reported with the DMR due April 28.) The results shall be <br />submitted on the Chronic Toxicity Test report form, available from the <br />Division.
