PART I <br />Page 2 <br />Permit No. COG-850000 <br />A. COVERAGE UNDER THIS PERMIT <br />1. This general permit authorizes discharges that consist entirely of <br />precipitation-induced surface runoff from coal mining and process in; <br />operations and Loadoucs, while active mining is cak'_ng ~?ace or duri^, <br />reclamation, into vacers of the SCate of Colorado. Exceptions are <br />those facilities in tie Cherry Creek Basin unscream of C::errv Cree:c <br />Reservoir and those tributary to Chatfield Reservoir downstream o: the <br />USGS gage at Waterton or in the Plum Creek drainage. No mine <br />drainage, preparation plant •aacer, spoils spring avatar, domestic <br />sewage or ocher wastewater other than surface runoff may be <br />discharged. Multiple discharges from one facility may be authorized <br />through this permit but P.LL DISCHARGES FROM A FACILITY COVERED BY T.!;IS <br />PERMIT MUST CONSIST ENTIRELY OF SURFACE RUNOFF. If a facility has <br />discharges from one or more of the above excluded sources alone or '_.. <br />addition co surf acs r~inoff discharges, an individual permit is <br />required. <br />2. In order to apply for certification under this general permit, the <br />owner, operator, and/or authorized agent of the subject facility shall <br />submit by certified mail or hand delivery, the completed Coal Mining <br />Wastewater Discharge Application (furnished by the Division) which <br />reouires the following information (at a minimum): <br />a) Nacre, address and descriptive location of tie facility; <br />b) iJame of principal in charge of operations; <br />c) Name of the water receiving the discharge; <br />d) Description o£ the activity including the anticipated duration oc <br />activity and/or discharge, anticipated volume, and rata of <br />discharge; <br />e) Description of any waste water containment and/or treatment; <br />f) A map showing the general geographical Locacien of the facilit_; <br />and a sketch o£ the facility showing oucfall(s) and receive..^.g <br />water(s); and <br />,) n analysis of the vacar to be discharged. <br />AC least thirty (30) days prior co the anticipated dace of discharge, <br />such information shall be submitted to: <br />Colorado Deoartmenc of Health <br />crater Quaiit-i Control Division <br />Permits and Enforcement Section <br />4210 East 11th avenue <br />Denver, Colorado 402.0 <br />The Division shall have up to thirty (30) days after receipt of tie <br />above information to request additional data or deny tie auchorizscion <br />to discharge. Upon receinc of additional information the Di~r_s on <br />shall have an additional :0 days to issue or deny autho[izacion _~ <br />disciarge. <br />
