Corrective Action; at Natural Soda <br />Inc; UIC Class III Permit #C030358-00000; Rio Blanco County <br />Dear Mr. Phillips: <br />Enclosed please fmd NSI's review of all wells listed in Tables 2 & 3 of the Final UIC <br />Permit, per corrective action required on page 12. First, attached is a summary table <br />which "identifies into which mining cavities each well is completed". The attached table <br />utilizes the most current status listed on Table 1, page 2 of the 2-19-04 permit <br />modification. This includes all injection/recovery and subsidence wells. Monitoring <br />wells aze not completed into cavities and monitor multiple cavities, <br />Second, attached are well schematic or diagrams (38) for each well listed on Tables 2 and <br />3 of pages 12 and 13 of the permit as required. Please note the following abbreviations: <br />MD =measured depth, Prod =production, ESP =electric submersible pump and BOM = <br />bottom of motor. <br />Third requirement of the permit is to make recommendations for addressing the repair <br />and use of any wells not properly cemented or completed as required by Part II (A) (2) of <br />the permit regazding "details of the solution mining injection wells". Although the focus <br />is on injection wells, NSI has reviewed all wells. <br />Infection Wells- All injection wells (4-3H, 4A-4V, 3A-4H, lA-4HI, SH, 6H, 2B-3C) have <br />cement to surface and meet all requirements for Part II (A) (2} of the permit. Initial <br />injection wells 90-SH, 91-2H and 93-4H, which had minimal cement and did not meet <br />Part II (A) (2), have been plugged and abandoned. We112B-3C has been scheduled for <br />plugging and abandonment (P&A) in 2004 or 2005. <br />/V~" <br />
