PART I <br />Page No. 8d <br />Permit No. CO-0000132 <br />B. MONITORING REQUIREMENTS <br />1. Freauencv and Sample Tvce (continued) <br />If the permittee, using the approved analytical methods, monitors any parameter more frequently than required by this <br />permit, then the results of such monitoring shall be included in the calculation and reporting of the values required in the <br />Dischazge Monitoring Report Form (DMRs) or other forms as required by the Division. Such increased frequency shall <br />also be indicated. <br />Outfall 007 Monitoring Exemption: The permittee is exempted from monitoring at outfall 007 during periods when <br />access would be hazazdous to personnel. For each reporting period that an exemption is requested, the permittee shall <br />provide written documentation of the conditions and specific periods during which access was restricted. The Division will <br />review this documentation and determine if an exemption is justified. If safe access is possible during a portion of a <br />reporting period, the permittee shall inspect the dischazge at the specified frequencies. <br />(d) Oil and Grease Monitoritta <br />For every outfall with oil and grease monitoring, in the event an oil sheen or floating oil is observed, a gab sample <br />shall be collected, analyzed, and repotted on the appropriate DMR. fr addition, corrective action shall be taken <br />immediately to mitigate [he dischage of oil and grease. A description of the corrective action taken should be <br />included with the DMR. <br />2. Six Time Analysis - Outfa11015 <br />The permittee shall collect six sRab samples at outfal1015 on a monthly basis, beginning on commencement of the new <br />discharge. The pazameters to be monitored aze: total recoverable azsenic, total recoverable cadmium, dissolved iron, and <br />dissolved manganese. <br />As soon as the results of all six analyses aze available to the permittee, the restilts shall be tabulated in a letter and <br />submitted to the Division. The due date for submission of this compliance item is September 1. 2000. If sampling is <br />delayed for reasons beyond the permittee's control, such as a period of no dischazge within the designated sampling period, <br />the perttilttee shall notify the Division in writing, indicating the reason for the delay and the adjusted projected submission <br />date. <br />See Part I.C. for Detinitioos. <br />Amendment No. 5 Issued Effective <br /> <br />
