Uttany <br />Division Director <br />Natural Resource Trustee <br />This review is conducted to determine compliance with Rule 4.05.9(17). The subject quarterly <br />inspections reports, dated 4/6/05, were on file at the mine. They are usually submitted to the <br />Division during the following quarter, but arrived on October 5, 2005, under a letter dated <br />October 4, 2005. The later-than-usual submission was reportedly due to personnel changes at the <br />mine along with associated confusion). <br />Inspection reports were submitted for Ponds B, C, D, E, F, G, T, the FCT, and the FCBH. No <br />reports were submitted for the pit pond, thickner underflow holding pond, 18-Right cuttings <br />impoundment, the evaporative impoundment at the Fish Creek Tipple, and the sewage treatment <br />ponds. These features were recently recognized as impoundments. This report was submitted <br />along with the 3rd quarter inspection reports. See comments to the operator in the response to that <br />report. <br />Were the ponds inspected for structural weakness? Yes [included differential settling, cracks or <br />slides, seepage, erosion (included rills and gullies on embankments, inadequate vegetation, outlet <br />channel erosion, burrows and other)]. <br />Were the ponds inspected for other hazardous conditions? Yes (included defective spillways, <br />dewatering device clogged, faulty gate, baffles, pumps, and other). <br />Did a qualified person designated by the operator prepare the reports? Yes, Sandra White, and <br />Brian Watterson, qualified persons, accomplished the inspections. <br />The reports also included information on sediment levels and any required maintenance.
