G. Everist, Inc., addressed one adequacy review item -the <br />decision date, with a request for an extension of our decision deadline to July 12, 2001, and noted <br />that all adequacy review items would be fully addressed in later correspondence. We now <br />respectfully submit the additional information requested in your Adequacy Review of May 30, 2001. <br />Item 1 -Signatures on maps [Rule 6.2(2)] <br />A copy of the maps have been signed by a qualified person from Banks and Gesso, the <br />consulting firm who prepared the maps and application for L.G. Everist. The maps are being <br />sent to the Division of Minerals and Geology (DMG) under separate cover, directly from Banks <br />and Gesso. <br />Item 2 - Backfilling of structural fill material [Rule 3.1.5(9) (a) through (f)] <br />Following is a short narrative concerning the use of imported inert materials to be used within <br />the permitted area as backfill, as per Rule 3.1.5(9): <br />(a) Location - As stated in the original application, L.G. Everist plans to import inert fill material <br />to backfill one section within the permitted area, labeled as Area A (see Exhibit C-3), which <br />is approximately 6.5 acres. We plan to mine this area for the sand and gravel reserves, <br />then backfill the area with imported inert fill materials and excess native overburden. <br />(b) Volume - In Table L, Reclamation Cost Estimate, on page 24 of the application, we have <br />stated the quantity for "Backfill to grade (Area A)" to be 360,000 cubic yards (or 540,000 <br />tons). Some of the backfill will be imported inert material, some will be native overburden <br />and non-marketable native aggregate. <br />(c) Affidavit certifying the material is clean and inert -Any imported backfill material to be <br />used on site will be inert material, as per the definitions in both the DMG rules and the <br />Hazardous Materials and Waste Management Division rules. <br />LMiLOH-adgreview-dmggs-060801 -via Hand Delivery <br />
