Only ouffall 022 discharged during this reporting period. No permit limitation exceedences occurred during this quarter. I did not have the current DMR form available so 1 just used a completed prior form and copied it and crossed out the prior data, or if it allowed just wrote over the prior data. In view of the fact that this report had to be submitted as soon as possible I thought this was the best way to handle the problem of not having a current DMR form. Due to a communication problem the report was overlooked and not forwarded to the Division. The problem has been addressed and should not arise again.. The problem was associated with a stalling problem and that issue has been resolved by that person leaving the company. A replacement for this stall member has been hired who will assist in preparation and review of the DMR's before the are sent to the Division. Chronic Whole Effluent Toxicity (WET) testing was performed at Outfall 022, indicating no toxicity to P. promelas or C. dubia. Copies of the CDPS WET Test Report Forms for both species are included in the submittal. Outfalls 003, 004, 007, 009, 011, 012, 013, 014, 015, 016, 017, 018, 019, 020, 021, 024 and 023 along with Site 3001 did not discharge, and therefore have no chemical or toxicological tests during this period. In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover the ouffalls which did not discharge.
