Description of Mininci Activities <br />This is a reclaimed underground mining operation. The mine <br />was recontoured and seeded with native grasses and shrubs <br />during 1986. All support buildings and facilities were <br />removed from the site during 1986. There is an access road to <br />the reclaimed area and there are three sediment ponds on the <br />site. <br />The mine is located approximately 5.5 miles northeast of <br />Paonia, Colorado. Access to the mine is provided by the <br />Hubbard Creek Access Road, off of old State Highway 133. <br />The mine permit area encompasses 340 acres. A total of 20 <br />acres were disturbed. <br />2. Area 5ubiect to Effluent Limitations Guidelines <br />The area subject to effluent limitations guidelines is nearly <br />the entire reclaimed area. The are no facilities, structures <br />or activities that occur within the reclaimed area. <br />3. Site Map <br />The site map is enclosed. <br />4. Description of Potential Pollutant Sources/Material Inventory <br />a. No materials are or have been treated, stored or disposed <br />of in a manner to allow exposure to stormwate~r discharge. <br />b. No spills or leaks of toxic or hazardous pollutants have <br />occurred at the facility. <br />c. The stormwater generated by this mining operation will <br />not potentially contain any pollutants except sediment. <br />d. No existing discharge sampling exists for stormwater <br />discharges. <br />5. Stormwater Ouality Controls <br />' a. SWMP Administrator: The individuals withi~i the mining <br />organization responsible for developing the SWMP and <br />implementation of the plan are Jim Stover and L. Kirt <br />t Jewkes with J. E.
