As a result of inadequate cement, flow down the <br />annulus was noted by temperature logs and tracer test. Additional cement work was <br />performed in April 2004. NSI observed positive results of the cement job, on the <br />mining injection/recovery flows. Logging of the 3M-TDR well was performed in <br />May 2004 and confinued positive results of cement jobs. Fill material from <br />cementing, limited logging to above 1630 FFS. May 2004 temperature and CBL <br />logs indicate 3M-TDR had been successfully cemented and the Dissolution Surface, <br />B-Crroove, A-Groove and Perched aquifers have been isolated in 3M-TDR. NSI <br />made commitment in May 12, 2004 letter to regulatory agencies to clean-out the <br />lower portion of 3M-TDR and obtain more complete logs during the Fall P/A <br />activity. <br />This letter was prepared to update regulatory agencies on the recent logging of 3M- <br />TDR, after drilling out bottom section of 3M-TDR- Contractor drilled out five foot <br />bridge at 1650', and ran bit to 1824' (solid cement). Logging was performed to <br />1786' where debris limited further logging. Due to perforation at 1650', NSI could <br />not circulate and clean-out bottom of hole. NSI will use a bailer to clean-0ut <br />remainder of casing to solid cement. Tagging the cement at 1824' and the presents <br />of debris indicates the bottom of the well is intact. See attached Sundry Notice and <br />well work summary. <br />
