V lam" <br />I have been further reviewing the application materials for this <br />permit, and am finding that most of the required items are <br />adequately addressed. However, there are still a few things that <br />will need to be clarified, as explained below. / <br />1) Any surface operations affecting lands previously undisturbed „ <br />must have a cultural resource survey performed, or must obtain ~~, <br />clearance for other mitigating activities, through the State <br />Historic Preservation Officer (SHPO). If you have questions, <br />contact Mr. Jim Green at the Colorado Historical Society, 303/866- <br />4674. No mining can begin until .the Division receives proof of <br />such clearance from SHPO. <br />2) In Exhibit N, the list of permits and licenses to be applied <br />for by the applicant should also include the NPDES (for discharge <br />of water) and a Storm Water Management Plan (SWMP), both of which <br />are obtained through the Colorado Department of Health/Water <br />Quality Control Division (WQCD). Based on information contained in <br />the application, the Division feels that stormwater runoff will be <br />an especially important item to plan for. This would be for both <br />the permit area and any offsite damage, too. Additionally, the / <br />applicant should consider groundwater quality and impacts such as / <br />accidental spills of fuel, oil and antifreeze, depth to / <br />groundwater, geologic permeability, etc. However, if the applicant ~' <br />U <br />feels that surface runoff or groundwater will not be negatively ~ <br />impacted, statements to that effect should be included. Inquiries ~ <br />may be directed to Ms. Pat Nelson at WQCD, 303/692-3590. The <br />Division encourages you to make this contact, to see if either <br />permit is required for your operation. <br />3) The reclamation detailed in Exhibit E mentions the stockpiling / <br />of topsoil/subsoil. Please provide the locations and quantities on / <br />a site plan for each phase. Push distances are essential for our <br />calculation of reclamation costs, and determination of plan b~. <br />compliance during mining. <br />
